Puppy Class Liability Waiver & Release

No Dog Too Small Dog Walking & Pet Sitting, LLC — Haverhill, MA

In consideration of being permitted to participate in puppy group classes, private lessons, or related activities ("Classes") offered by No Dog Too Small Dog Walking and Pet Sitting, LLC ("the Business"), I, the undersigned owner/handler, agree to the following:

1. Assumption of Risk

I understand that working with dogs — including puppies in a group or individual training setting — carries inherent risks, including but not limited to bites, scratches, trips or falls, property damage, and the transmission of illness between animals. I voluntarily assume all such risks for myself, my puppy, and any family members or guests I bring to Classes.

2. Health & Vaccination

I confirm that my puppy is, to the best of my knowledge, healthy, free of contagious illness, and current on age-appropriate vaccinations or veterinarian-approved titers/protocols as verified in the intake questionnaire. I agree not to bring my puppy to class if they are showing signs of illness (vomiting, diarrhea, coughing, lethargy, fever, or known exposure to a contagious disease) within 7 days of a scheduled class.

3. Behavior Disclosure

I have disclosed, to the best of my knowledge, any history of aggression, resource guarding, fear-based behavior, or bite history involving my puppy. I understand that undisclosed behavior history may affect my puppy's suitability for group class and may result in a recommendation to switch to private lessons at the instructor's discretion.

4. Release of Liability

To the fullest extent permitted by Massachusetts law, I release, waive, discharge, and covenant not to sue the Business, its owner, employees, and assistants from any and all liability, claims, demands, or causes of action arising out of ordinary negligence in connection with my participation, or my puppy's participation, in Classes. This release does not apply to injury or damage caused by gross negligence or willful misconduct.

5. Indemnification

I agree to indemnify and hold harmless the Business,No Dog Too Small.Dog Walking and Pet Sitting, LLC. from any claims, damages, or expenses (including reasonable attorney's fees) arising from injury or damage caused by my puppy to any other person, animal, or property during Classes.

6. Photo/Video Release

I give permission for photos/videos of my puppy taken during class to be used for the Business's marketing, website, and social media.

I do NOT give permission for photos/videos of my puppy to be used publicly.

7. Handling Authorization

I authorize the instructor and assistants to handle, leash, and, if necessary, physically manage my puppy during class activities for the safety of all participants.

8. Emergency Veterinary Care

In the event my puppy is injured or becomes ill during class and I cannot be immediately reached, I authorize the Business to seek emergency veterinary care on my puppy's behalf. I understand I am responsible for all resulting veterinary costs.

9. Acknowledgment

I have read this waiver and questionnaire in full, understand its contents, and sign it voluntarily. I am at least 18 years of age and the legal owner or authorized agent of the puppy named above.
